School Year Application:  Used to apply to the 2008-2009 school term.  Applications accompanied with the registration fee can be sent to 1588 South Victoria Road, Mendota Heights MN 55118.

Summer Camp Application:  Used to apply for the 2008 "Summer on the Farm" program.  Applications accompanied with registration fee can be sent to 1588 South Victoria Road, Mendota Heights MN 55118.

Immunization Record:  We are required by law to have a completed Immunization Record on file for every child in attendance at Children's Country Day School.  This form may be filled out and signed by a parent or guardian if the child is at least 15 months old and has had all the shots required by law.  If there are medical reasons why a child can’t have or doesn’t need any shot or shots, a doctor or a public health nurse must sign in Section B.  If your child is not up to date on his or her shots, by law you have 18 months after enrolling for your child to have all of his or her required shots. 

Health Summary:  To be completed by a health care source and required for every child attending Children's Country Day School. 

Permission Form:  To be completed by parent or guardian giving a total of six permissions and required for every child attending Children's Country Day School.

School Questionnaire:  To be completed by parent or guardian covering a variety of information in order to help your child's experience at Children's Country Day School be more successful.    

Ointment Permission Form: To be completed by parent or guardian to give Children's Country Day School permission to apply ointment, namely diaper cream.

Medication Authorization Form: To be completed and signed by both Physician and Parent or guardian giving permission, instruction, and direction for Children's Country Day School to administer medication both prescription and over the counter.
